Office Preparation
When you come to our office, please remember to bring the following:
- Medical Insurance Card(s)
- Medications (You must list ALL medications you are taking incliuding eye drops, nose sprays, respiratory inhalers and patches. If you are uncertain of the name or dosage, please bring them with you to the visit. Also include any vitamins and herbal supplements).
- Your insurance co-pay or other required payment for services.
It is very important that you complete the forms given or mailed to you accurately and bring them with you at the time of your visit. If you do not have the required forms and are a new patient, please arrive 10-15 minutes earlier than your scheduled appointment time in order to complete them.
All co-payments, self payments and payments for non-insurance covered services are required to be paid in full at the time the service is rendered or a $25 administrative fee will be added to your account. If you are unsure of these charges, please contact our billing office in advance of your visit.
There will be a $25 charge for any no-show office visit and a $50 no-show charge for any office procedures.

Please call us to see if your plan is acepted and to find out whether a referral from your Primary Care Provider is required.
Our practice strives to be in compliance with the Federal Health Insurance Privacy and Portability Act (HIPAA) regulations. If you have not done so, you will be asked to review and sign our Notice of Privacy Practices. Please ask one of our staff members if you have any questions.
